Provides that a worker shall (for the purposes of section 69 of the Workers' Compensation and Rehabilitation Act 1981) prove his identity and the continuance of the incapacity in respect of which a weekly payment is payable, by delivering to the employer or the employer's insurer, at intervals of three months, a declaration by the worker and by a medical practitioner. Provides for requests for reference to a medical assessment panel and proceedings before a medical assessment panel and related matters. Made under the Workers Compensation and Rehabilitation Act 1981.
